Please complete this form for each student to attent Maryland Talent classes, camps, or lessons. The information provided below will be used exclusively by Maryland Talent for the safety and benefit of the student.
It is only necessary to complete this form once per student.

Make all checks payable to MDTEC. Once your deposit is received, your private teacher will contact you to schedule your day/time for private lessons.
The first payment for private lessons is due by August 24th. Group payments are due at the first group class.
